Where are you from?  (formal/polite)

Literally From where are you?  Remember this is the form to use when usted is the subject.  Usted is the formal or polite form of you that is used to show respect, i.e. adults, authority figures, your elders.
 

Soy de Pennsylvania. 

I am from Pennsylvania.

Notice that there is a change in the verb and the subject from the question to the answer.

Literally From where is she/he?
 

Es de Pennsylvania. 

He/she is from Pennsylvania.

Notice that there is not a change in the verb, because there is not a change in the subject from the question to the answer.
